Be prepared to provide us the following information. This checklist is provided for your convenience in gathering information for reporting your loss. Remember, you will want to report losses as soon as possible, so don’t worry if you don’t have all of the answers.
- Your name, phone number and policy number
- The date and approximate time that the incident occurred
- The location where the incident occurred
- As applicable:
- The description of the other car(s) involved
- The name of the other driver(s) involved
- The reporting police agency and report number
